To make the most of your appointments, we request that you fill out the required agreement and intake forms that apply to you prior to your visit. Doing so will also help you better understand the nature of the therapy you will be receiving, along with its benefits and risks. Rest assured that your information will be kept confidential.

I do bill some third-party payers (e.g., Canadian Armed Forces (CFB) (Medavie Blue Cross); Veterans Affairs (Medavie Blue Cross); RCMP (Medavie Blue Cross); and private plan (Pacific Blue Cross). Please contact me if you have any questions.
